What is your current height and weight?
My height is 5'4" and my current weight is 152 lbs.
What would your ideal weight be?
My ideal is around 120 lbs.!
At what weight would you like to be 4 months from now?
Ideally, I'd like to be at 120-125 lbs. ....?
Losing weight for a specific event? If so, when is the event?
Nah. Not losing weight for an event. Was losing weight for my birthday, but that's coming up on the 26th, and I'm not going to be able to make that goal
Possible obstacles that could get between you and your weight loss goals?
My mom's cooking, eating too much, exercising too little.
Why do you think that you now have a weight problem?
Emotional eating, I think!
Lifestyle changes that you feel will help you lose weight?
Exercising more, and eating different things/less.
Have you lost weight in the past?
Yes. I was obese as a teenager, and lost 100 lbs. Then more recently... about 50 lbs. off of 170 lbs, but I gained it all back. Now I'm back to losing!
Why do you believe that you gained the weight back?
Lack of exercising. Eating too much.
What has not helped you lose weight in the past and why do you think it didn't work?
Eating less and eating the right thing. Exercising more.
Would you try writing down all food and drinks that you consume?
Yes I would. That has helped tremendously in the past, now that I think about it.
How often and what do you cook at home?
I just make my own breakfast and lunch. We have a family dinner where my mom cooks.
How often do you go out to eat and where do you go?
I hardly ever go out.
What are your 3 favorite foods?
Yogurt

What are your 3 favorite restaurants?
I hardly ever go out, so I can't think of any
Three things you can do differently when it comes to food?
Eat more fruit
Eat more veggies
Eat less starches in general.
If you woke up tomorrow and your body was exactly the way you want it, what would be different?
I would be about 115-120 lbs.!
Do you eat when you are not hungry?
I guess so... I try not to. I try to drink water instead.
Do you binge eat?
Nopers.
Do you hide your food or eat in secret?
I don't like to eat in front of people, but I don't binge eat in secret or eat bad foods or anything like that... it's pretty healthy.
Do you eat when you are sad, nervous, or depressed?
I think I do. I think that's why I got as heavy as I am.
Do you eat as a reward?
Nopers.
Do you eat while watching TV or using the computer?
TV, yes, computer, no. I know, we shouldn't have dinner in front of the television, but we've just gotten used to it, I guess
What do you normally eat for a meal?
A starch and a fruit, as I'm finding out...
What type of snacks do you eat?
I like FiberOne or FiberPlus bars... anything with fiber in it, I guess...
What are you currently doing exercise wise?
I have an exercise class that I go to twice a week, and I exercise every morning for half an hour with the Wii Fit, doing step aerobics. I also like Wii Sports Bowling.
Where do you go for exercise?
To my exercise class, or hehe... my living room

Step Aerobics
Yoga
Strength Training (which I need to do more of!)

Nah, I'll just be able to fit into nice clothes again!
