weights first - cardio second

Can someone please advise if this is the best format
Do 5 min warm-up, followed by weight training, and then finally cardio
 
Yes, that's good. Another option is to do them at separate times of the day, though that can be inconvient if you go to the gym for both.

It's important to go in to your weight training with your energy stores full, which is why it's better to wait to do cardio after weights.
