Everyone should check out the Squat RX link. He troubbleshoots the squat. Look through the list and see if you find some of your problems, like GMing out of the hole, heels coming off the floor, etc. He also shows a lot of supplemental exercises to help your squats (and deads).
So everyone, nomatter how advanced you think you are, check it out.
