I think every few months is better. It'd be too much hassle changing every week and to a certain extent you do need repetition in a workout programme, but as well as some diversity to avoid plateuas (sp). So I'd say every month or so.
 
My routine changes weekly. For example-I change out front squat, back squat, good mornings, and deadlift for my main leg/posterior end work every week. On my lower body days, I change out the clean for the snatch and visa versa. My volume also changes weekly, so it's really about find what's best for you.
