Putting together a workout

Hi,
I'd like to put together a work out plan, but other than setting my goals I'm not too sure where else to start. I used to be in great shape (a lean 190) and I'd like to get back to it. I'm used to intense work outs but I have a weak right knee and a weak right achilles tthat I'd like to strengthen. My goal is to work up to an intense workout while strengthening my knee and my achilles.
I used to run for about 25 minutes on treadmill with speed intervals ranging from 6 to 8.2 mph and varying inclines. Unfortunately I can't remember to what levels.
Any help or ideas would be greatly appreciated.

The elliptical machine has been great on my weak knees. Really helped to strengthen my thighs and the other muscles around my knees. I'd try to work with the elliptical for your cardio and just try to avoid knee-hurting weight machines or postures.
