Progressive Overload

If I'm using the progressive overload method when I lift dumbells should I lift to muscle failiure on every set or only on the last set of my routine?

also, should I opt for a low rep/high weight exersize when I train for muscle failiure or do the opposite and go for high rep, low weight. I'm training for size and strength if it matters.

for size and strength, I would never go high rep / low weight.

typically you only go to failure on the last 1-2 sets. you'll need a spotter for many lifts to failure, and keep in mind that lifting to failure all the time is very hard on your nervous system, and isn't advised to do every session.
