Online Guide/Journal

Does anyone have a site where I could download or print a guide and or journal I could keep track of my progress in? I remember in high school, we had thse bfs (bigger, faster, stronger) books/journals that gave us step by step, day by day instruction on reps/sets and we recorded the weight we used in each little box.
