QUESTIONAIRE
-- How much weight do you want to lose?
~60 pounds
-- What is the time frame for reaching your target weight?
June 12, 2009. My graduation date!
-- How do you want to accomplish your goal (what methods do you want to use)?
I'll use my plate and divide it into three sections, with the vegetable portion being the largest. I will eat many mini meals/day. I will exercise frequently and always take the physical approach.
-- How realistic is your goal?
Not too realistic, because I don't have the time to devote to lose 10 lbs/month.
-- When will you start?
Tomorrow =)
1. What is your current height and weight? 5'5" ++++ I'll fill in the weight once i come back from the gym
2. If you were at an ideal weight now, what would that weight be? 125
3. At what weight would you like to be at four months from now? 140
4. Why do you want to lose weight? To feel healthier. As I have gained ~20 pounds in the past 10 weeks, I feel all the aches and pains with it...gosh, no more!
5. Do you want to lose weight for a specific life event such as wedding or reunion? If so, when is that event? My college graduation!
6. What obstacles could get between you and your weight loss goals? Laziness. Stress. Time commitment.
7. Why do you think that you now have a weight problem? Because I let my self-destructive behavior take over and act like I don't care, when in fact I just am really sad about my life.
8. What lifestyle changes do you think would help you lose weight? To be more positive and be around positive people.
9. Have you lost weight in the past? Yes, I've been low to 165 and high to 198.
10. Why do you believe that you did not lose weight or you gained the weight back? I just love food.
11. What, if anything, has not worked for you in the past in helping you to lose weight? Why do you think it did not work? Being all or none.
13. Do you cook at home often? If so, what do you cook? I cook pasta, or meats, or egg/ham/cheese sandwiches. I don't cook fancy foods with recipes and all, but i do know how to make chicken breast tender.
14. How often do you go out to eat? Where do you go? I used to go out often. I try to eat at home.
15. What are your three favorite foods? Super steak burrito. Spicy Pork Nachos. Egg/cheese/ham sandwiches.
16. What are your three favorite restaurants?
Mexican Taquerias, Student Coho/Bakery, Anything Fatty
17. What are three things you can do differently when it comes to food? Drink water before consumption of solids, eat a portion, not the whole thing, FOOD LASTS LONGER IF EATEN IN PORTIONS,
18. If you woke up tomorrow and your body was exactly the way you want it, what would be different? my mood
19. Do you eat when you are not hungry? of course. Hopefully no more.
20. Do you binge eat (large amounts at a time)? yes
21. Do you hide your food or eat in secret? yes
22. Do you eat when you are sad, nervous, or depressed? yes depressed cause i eat or i eat cause i'm depressed? it's the chicken or the egg kind of thing.
23. Do you eat as a reward? I have no rewards right now.
24. Do you eat while watching TV or using the computer? All the time.
25. What do you normally eat for a meal? I usually eat as much food as for two people. Whatever it is, I eat twice the amount.
26. What type of snacks do you eat? Not too many snacks, more like meals.
27. In terms of exercise, what, if anything, are you currently doing? Nothing. I am recovering from tendonitis.
28. Where do you go for exercise? A local public gym? School/work gym? Home? I have a membership to the 24hr fitness AND to my school gym.
29. What, if anything, are your three favorite types of exercise? elliptical climber, not trainer. Weight lifting, and dancing!!!
30. What is your daily/weekly/monthly/yearly motivation to move towards your goals? How I will feel, and that will affect how I view life.
31. Do you have rewards for certain goals? Not in particular. Just more HAPPINESS
