How to bulk and how to take measurements of arms, chest, etc.

I was wondering how bulking is done. I am 16 years old, 5'8" 123 lbs. I am not sure wether i should use creatine and how many reps i should do of each exercise (low reps with more sets or vice versa). Also, where do you take the measurements when you want to know size of arms, legs, chest, and stomach, and forearm.
