Burn out or not?

I have been lifting to bulk up and wondering if i should be going to total failure? By this i mean should i just get a little help with the last rep or should i do a couple more with help, when is enough?
 
A little failure is cool but failure all the time is immensely hard on the body and often leads to inadequate recovery. An example is perhaps going to failure on your last set but not every single one.
