Please help me create a fitness routine!

Hello, I am new to this website because I need some fitness help. I do not want to lose any weight, I am looking to get VERY fit and toned. I am 5'8 and weigh 140 lbs

My calves, thighs and tricpes have no definition; they're very jiggly and I don't think they look nice.

How often should I be working out in a week? And what should I be doing at the gym? Also, what types of foods should I be eating before/after the gym? Anyone have any good tips to get toned fast? Any success stories? My weakness is sweets, I like chocolate very much.

I also might be going on vacation in one month and I want to see a difference by then.

Any help would be greatly appreciated, I need some assistance with direction, I'm having trouble figuring this out on my own and I cannot afford a personal trainer right now.
