If I understand correctly, in the past 14 weeks you have worked out every single day for 1.5 to 2 hours per day doing 15-18 machines and 6 sets on each machine????
If I were you, I would dramatically reduce the time spent in the gym. Using 15-18 machine is inefficient and not necessary. 45 minutes of weight training gives you plenty of time for a very intense training. Make sure you are increasing the weights that you use whenever you can. You need to train HARD.
You are not doing your body a favour by never taking a day off. Build in 2 rest days per week. Your body needs rest. Remember that muscle don't grow in the gym - they grow when you rest.
As for the cardio, I agree with the others, increase the intensity, do something different than usual.
Actually, after 14 weeks of non-stop training you should consider taking a whole week off. Your body will love it and you will get better results afterwards.
