Recurring Shoulder Problems

My shoulders give me pain when working out. Ive had enough, and want to fix them for good. My left one was seperated and my right one was sprained.

Today I did this work out for my shoulders.

Single Arm Laterials:
Set 1: 15lb x 15 Set 2: 15x12 Set 3:15x10 Set 4:15x8

Bent-Over dumbell Laterial Raises
Set 1: 10x15 Set 2: 10x 12 Set 3: 10x10 Set 4:10x8

Side Lateral Raises:
set 1:10x15 set 2:10x12 set 3:10x10 set 4:10x8

Dumbell Shoulder Press:
Set1:10x12 Set 2:15x10 Set3: 25x8 Set4: 25x6
(Note: I felt much more comfortable performing this exercise than the Laterials)

Behind the Neck Shoulder Press:
Set1:30x15 Set 2:40x10 Set3:50x8 Set4:50x6
-I also felt much more comfortable doing this exercise. It really hurts when I do the laterials.

What would you guys recommend? It seems like I was experiencing difficulty with the laterals much more than the other two, so maybe thats the particular area I need to rehab. Any advice guys? Thanks
 
The shoulder is the most movable joint in the body. However, it is an unstable joint because of the range of motion allowed. This instability increases the likelihood of joint injury, often leading to a degenerative process in which tissues break down and no longer function well.Shoulder pain may be localized or may be referred to areas around the shoulder or down the arm.

I get recurring rotator cut issues in my shoulders from swimming (I am an Ironman triathlete) and from weightlifting. I am not a medical professional, but I would advise you to seek professional advise from an orthopedist before proceeding. "Working through" a rotator cuff issue can be very problematic.
