Protein Sparing Modified Fast (PSMF) - 315 to 150lbs

I'm not usually the type of person to make a thread about something like this, but I need to keep track of what I'm doing, and this seems the best way (so others may learn from my knowledge/mistakes).

I was about 330 - 340lbs in September (my scale wasn't too great, so I don't know precisely), went on the Atkins diet for about 6 weeks and lost down to 310lbs. Went off the diet and gained to 315lbs (water gain), where I've held steady these last few months. Now that I've gotten past the issue that made me go off the diet, I'm ready to attack this issue head on with a protein sparing modified fast (PSMF).

I bought a new scale, which is very accurate, but went to the Dr. anyway to verify my weight and blood pressure, which was 315lbs on March 8th (two days ago), and 120/80 blood pressure (pre-hypertension). I started the diet on March 9th.

As of right this moment (9:45PM March 10th) the scale shows 311.6lbs - the rest of my weigh-ins will be done in the morning. I haven't exercised these two days, but will once my body adjusts completely.

Waist - 62 inches (5.1 FT!) circumference.

ABOUT A PSMF DIET

This isn't a way of eating for life - this is battle mode for those who need/want to strip off a massive chunk of fat. I need/want to. I've never been thin a day in my life - I weighed 120lbs at 11 years of age, 200lbs at 14, 250lbs at 16, etc. up to nearly 350lbs at 21 years old (my current age). I've never been on a date. I've never particularly enjoyed life. Every moment of my existence is marred by the discomfort I feel from being so large. My blood pressure is pre-hypertension. I've begun to experience blood sugar issues (extreme exhaustion after eating sugar/vision issues). Time to stop thinking about the day I'll lose the weight and actually do it.

A protein-sparing modified fast is basically a fast (starvation) with the inclusion of 1 - 1.5 grams of protein per pound of lean body mass + some fat + leafy veggies + supplements. The idea is that your body runs off fat or glucose. If you deprive it of both, your body will eat your fat for its energy needs and your muscle for its glucose needs. The problem with this is obvious - you're starving, and your body is cannibalizing itself. To remedy this, you eat sufficient protein for your body's need, and it will use your fat stores for your energy needs.

MY DIET

I'm following Lyle McDonald's Rapid Fat Loss diet, but the Cleveland Clinic also has a PSMF diet if anyone is interested in checking it out. Always consult with your doctor first, this is an extreme diet that requires you to have sufficient knowledge of biology/nutrition, your own body's limits and needs, etc. I'm doing a PSMF for six weeks with one 'refeed' a week.

I'm also taking 25mg of ephedrine and 200mg of caffeine twice a day. Less anyone panic about this, I monitor my blood pressure very closely, and have a high stimulant tolerance. I have asthma, so I've been taking ephedrine regularly over a long period of my life. I know my levels and what I can handle, but I don't know yours, and thus I advise you against using an EC stack. Talk to your doctor if you're interested.

I eat about 900 calories a day total, usually consisting of chicken jerky and baked fish fillets. I also eat cucumber slices and/or a green pepper and/or an iceberg-spinach salad. I take [1] multivitamin, [1] calcium, and [3] fish oil capsules in the morning, and again with dinner. I take some magnesium and potassium twice a week, and will increase/decrease it depending on how I feel in a week. Some people require a fiber supplement, but I haven't, and don't think I will, since I consume adequate veggies.

MY EXERCISE

Extreme or heavy exercise on a PSMF is not possible, and would be incredibly stupid. I don't feel tired or anything, but I can tell just moving around that I'll certainly not last long if I strenuously exercise. That's fine. Diet is 90% of weight loss, and, at my current weight, is more important in terms of pure weight loss than exercise (though exercise is certainly necessary).

My plans currently are weight lifting twice a week with one large protein shake (250 calories/56g protein/16g carbs) immediately before. The other days will be a leisurely 1 mile walk down a nearby nature trail, or something similar.

THE FOOD/PROGRESS SO FAR

Yesterday, the first day on this diet, was brutal. I felt fine until about 3PM, which is when I normally have my first real meal of the day. The later half of the day, up until about 11PM, was very difficult. I was cold and shaky and had a perpetual headache. My mind was fuzzy and I could concentrate on nothing. I was grouchy because I was hungry and had a headache. After the sun went down, I got terrible cravings for all sorts of terrible foods (you guessed it - I usually eat junk food around 9PM). It took a lot to power through it, but I did.

March 8th

[3] hamburger patties
[3] slices provolone cheese
[2] fish fillets
[1] whey protein shake (250 calories/50P-16C + 18 amino acids + essential minerals)
No veggies because I was out.

March 9th (today)

Today I felt much better. I did wake up hungry, though not ravenous - just a lingering sensation. No headaches or cravings at any point today. Though I still have an urge (from habit) to pop all sorts of junk food in my mouth, I actually feel physically good. Well balanced, if that makes sense. Not hungry even though I've had so little. Not dizzy, cold, moody. I have amazing mental clarity. I haven't felt exhausted at any point today, which is unusual, since I usually crash around 4PM and can hardly stay awake. It's currently 11PM, I arose at 8AM, and I still feel fine. It really is incredible.

[3] hamburger patties

[1] whey protein shake

Today was pay day, so tomorrow I'm going grocery shopping for a wider variety of food including veggies. My plan is to make a lot of chicken jerky, which I've made in the past, because: A) I love jerky, B) It's easy to snack on when the munchies set in, C) because I hate cooking.

I'll add pics at some point, though I can't guarantee when. I'll weigh in everyday and report on how I feel, and if I experience any health issues.

EDIT: After re-reading this, I'd like to toss in a few clarifications. I've made very minor tweaks to the RFL version of the PSMF, based on reasons I won't get into. I ate cheese these two days because I can't buy fish oil capsules until tomorrow, and certain vitamins in the multivitamin are fat soluble. There will be no cheese starting tomorrow. I'm also eating hamburger patties because I'm almost out of food, and all I had was a package of frozen hamburgers and some fish fillets. As I said, tomorrow I go shopping.

Why the rush? Why not implement slow changes into your lifestyle and slowly changing your lifestyle?

You'll enjoy this a lot more when you take it slowly. I know it's hard because you want everything NOW. You want to see change IMMEDIATELY. But honestly (and I'm sure a majority on the people on this board can attest to this) make slow lifestyle changes. Slowly cut your calorie intake. Substitute healthy foods. Slowly incorporate some kind of physical activity into your lifestyle. You'll find that you can stick to these kinds of changes for the rest of your life.

You'll enjoy this a lot more when you take it slowly. I know it's hard because you want everything NOW. You want to see change IMMEDIATELY. But honestly (and I'm sure a majority on the people on this board can attest to this) make slow lifestyle changes. Slowly cut your calorie intake. Substitute healthy foods. Slowly incorporate some kind of physical activity into your lifestyle. You'll find that you can stick to these kinds of changes for the rest of your life.

I attended intensive kickboxing lessons (as in, throwing up after every session sort of intensive) five days a week for three years, and while it did greatly increase my fitness, it scarcely put a dent in my weight. My weight is due to steroid medication, and it isn't going to come off without a fight.

Simply cutting calories and exercising is something I did for years without significant success. It's like throwing a feather at a brick wall. It's time to break out the big guns.
 
Morning weigh in = 310.4 lbs. (Yesterday = 311.6 lbs)

Didn't wake up feeling hungry today. Didn't need as much sleep last night as usual. Have that characteristic metallic taste in my mouth typical of ketosis, like sucking on a penny.

"Breakfast":

- [3] Fish oil
- [1] Ephedrine
- [1] Caffeine
- Bottle of Crystal Light.

Will update this post tonight with exercise and stuff.

No offense..but I'm wondering if you really did cut calories and exercise. The reason is that is weight loss..calories in vs calories out. I'm guessing you was underestimating the calorie count you was actually taking in.

I hope you will be ok with this..but jump starts don't help if you don't know how to keep it running.

I have a degree in biochemisty, and am analytical to a fault. My food/exercise diary from that diet is about 250 pages long, and I know precisely what I was consuming down to the tenth of a percentage. While basic thermodynamics always exist, losing weight can, and often is, far more complex than simple calorie intake/deficit if the weight gain itself was caused by something other than mere overconsumption of energy.

I won't get into the science of it all, but try taking extremely high doses of steroids everyday for a decade and you'll be amazed at the ways it reprograms your body. Simply cutting calories to lose that weight becomes a pipe dream.

Anyway, my point of this thread isn't to nitpick at my dieting choice - as I said, I have a degree in biochemistry, my decision to do a PSMF was based on many factors relevant to myself and my past, and my doctor is monitoring the entire process closely.
 
I just want to say good luck and I hope you see some good results! I love jerky! I have been meaning to make my own but I want that dehydrator thing before I actually attempt to make it. I want to do beef though or maybe try biltong because I heard amazing things about it.

Hoping you get to feel healthier results soon! I definitely think it takes major will power to do anything that asks you to all this stuff so wow. Also atkins!!! less then 20 carbs a day is just so extreme for me. I don't think I could ever do that but I guess we never know what we can truly do until we try to do it :)
 
Forgot to update for a couple days.

March 12th weigh in - 309.8

For dinner on March 13th, I had my first free meal, which was a fajita skillet at a local Mexican restaurant, and a plate of refried beans covered in chopped raw veggies (onions/peppers/tomatoes) topped with sour cream and sprinkled with mozzarella. It was very delicious, about 1200 calories, but was still low on carbs. I also had a small glass of red wine.

March 13th weigh in - 308.6

Despite my large dinner, I weighed in the next morning at 308.6 - I suspect the sudden glut of food prompted my body into a whoosh effect as it realized I wasn't starving. I was curious about what percentage of my weight was water/glycogen versus fat, so I ate 'normal' food on the 13th, and have so far today also had a normal diet, allowing my glycogen stores to replenish.

My weight seems to have stabilized around 312-ish, indicating that I lost about 7-ish pounds of weight, 3-ish LBS of which was fat, which is right in line with what is typical. I was finally able to go to the grocer today. I picked up two bags of chicken that'll be turned into jerky.

I'll upload some pictures of the jerky making process if anyone is interested. Tomorrow the diet resumes, and finally the snow and ice have abated, so I should be able to get out and exercise.

If you don't have a dehydrator, you can use a box fan to make beef jerky. I've used a fan many times, and it works well (for beef, not chicken). Just make sure your jerky marinade has salt and some type of acid (like a squirt of lime juice), which will inhibit bacteria growth while the jerky is growing.

Hi Lurk, I can't agree with you more. I share some similarities with you. I also viewed PSMF as a "bringing out the big guns" diet. I kept gaining weight from medications I take for bipolar disorder. On January 17th I started Lyle's RFL diet. I just completed my 2nd round today and I just started my second diet break. Thus far I have lost 22 pounds in 8 weeks.

Good luck and be ready for some surprises with the diet. You probably won't be hungry but you will have a lot of mental struggles and irritability with this diet.

Did you take steroids for a medical reason or for muscle building during your kickboxing periods?

Could you go into a little detail about how steroids can affect your body "make up" or if you dont want to dirty your diary just PM me instead.

You seam really motivated to do this, you have made a good start and im sure you will be able to do this. When possible try and come back a post everyday, you would be surprised how much it helps.
 
I keep forgetting to update this until I get one of those weekly updates in my email :p

I dropped a paint can on my scale and cracked the screen, so I don't know what I weigh at this point. I haven't been terribly motivated to get a new one since I know I'm losing weight.

To be frank, and since I don't know how else to describe it, my 'gut roll' has definitely shrunk. I really noticed the difference today when I leaned over the sink and my stomach barely touched it. My face also looks a little thinner.

In terms of side effects, there haven't been any except the occasional phantom craving for something bad and muscle weakness if I exert myself too much (like lugging a recliner up two flights of stairs).

My last free meal was another fajitas dinner at the local Mexican restaurant because it's so tasty and is naturally lowish carbs. I passed on the corn chips this time, but did have two flour tortillas.

Every now and again I eat something I'm not supposed to, but it hasn't drastically interfered with weight loss. I had a glass of milk earlier; a couple bites of pizza a couple days ago; etc.

I've been drinking mostly water, but had a cup of iced coffee with Truvia yesterday and a few bottles of Crystal Light over the week. Overall, I give myself a B+ in terms of sticking to the diet.

Will, once again, try to get picks up. I'll put a post it note on my bathroom mirror.

They were corticosteroids, mostly predisone, but a few others also. Basically the steroids reset your bodies metabolism so that it is very low while causing you to be so ungodly hungry that you could eat an entire house. They also cause the body to put fat in unfortunate places, such as the back of the neck and the face, resulting in what is affectionately known as the corticosteroid 'moon face'.

Your body will be very stubborn to up your metabolism and shed your steroid weight once you stop taking them. It basically sees your end weight and your low metabolism as being 'normal', and true normal as being underweight.
