Need some advice on training etc

I am 46 female. I started a diet on 19th Jan 2009 and now have lost 65lb. I wish to possibily loose another 11lb so i can get to my goal weight of 8 1/2 stone, I was 14st 2 approx. I am lucky in the fact I have not got that much loose skin. I started exercising on the same day as I started my diet.
I have regularly changed my workouts, going from 1 1/2hrs cardio and mchine weights 3 sets of 12 reps every time I go (at least 6 days a week) to 1hr cardio and 3 sets of 8 reps on 14 machines. The weight I now lift I struggle at the 6th rep, can just about manage the 8th with a grunt lol.
I am not sure of my body fat measurement as each time I ask the instructor they say yes and either forget or are not there :mad:
Hubby has just brought me a heart rate monitor and I have a really good x trainer at home. I have tried this HRM out and my fat burning rate is between 113 - 139 and I am struggling to get it to 110. I managed to get it to 126 by increasing the resistance as if I went faster it made no difference to my heart rate at all, which I find surprising, as I was sweating buckets!!! The HRM is a watch with a chest strap.
What am I doing wrong surely I don't have to take my body to near on heart attack to get into my fat burning rate?
My quest is to have a defined body, not body build but look fit and toned. I am getting there and hopefully now I have uped my weights I should be able to see some reaction at some point. How long do you think it will take me to see any improvments?
My diet consists of
Lean meat, fish, eggs, little cheese, all salad, all veg, all fruit, nuts, dried fruit and yogurt. I will have some lower fat sauces like extra light salad cream but don't go over the top with these. I now don't eat bread pasta rice or potatoes, and find this way of eating superb for me as I now never get cravings for sweet stuff. I really don't think I could include thse now in my diet, I tried on holiday and I felt dreadful.
I hope sombody may beable to help me with these few points that are driving me insane now. I am considering doing free weights too, what do you think?

Congrats on the 65# weight loss and good luck with the remaining 11#. You did it right - watch your diet, cardio, and weight training - the three components to weight loss. Yeah, you can lost weight with 1 or even 2 of these, but all three is the best way to go as you found out.

110 bpm for a 46yo woman is not a high rate - that should be a rate that you would get at a brisk walk. Are you sure it is working properly? I am a 50yo male and I would struggle to keep my heart rate below 130 during weightlifting or cardio workouts.
 
Hi thank you for your reply. The HRM that I have just got is pretty good from Decathlon, it seems to go up and down when wearing it. I just can't understand why on earth my heart rate wont go to a basic fat burning level unless I am pushing myself really really hard, on my x trainer 70 rpm at hardest resistance!!!. The reason I wanted a HRM was for the fact the gym where I go when I look at my HR on their monitors on the machines I too easy go into cardio training, so I thought a HRM would allow me to train within the fat burning level with a more exact reading. Either my HRM or the gyms are not working, otherwise could it be an indictation there is something else wrong?
Deep inside I know I should not have to push myself to the levels I have today to reach 110.
