Well it looks like you have some really good ideas of changes that you want to make, so I think you're off to a great start. By having a short term and long term plan for each resolution, you're setting yourself up for success! The only thing I could suggest is to start looking into #3 and maybe even sign up for something now that will start in January.
No ideas for #4 - you are planning to make some good changes already, so I think that's a great way to start the new year!
