Breaking the cycle

Im new to this so bear with me and my extended introduction. I find that my most successful dieting happens (strangely) when I do it on my own and don't publicize it, but lately I feel that having an anonymous group to share my progress with might help me stay on track. My entire extended family is overweight and it's always scared me to death to imagine my future like that. I'm only 5'2" and after hitting 167 pounds during college, I'd had enough. In 10 months, following a low calorie diet and occassionally exercise, I got down to 113 pounds. I'd do anything to get back to that weight. Over the past 2 years I've gained 16 pounds and am now at 129. I can't ever imagine being heavy again and already find myself retreating from social outings because im uncomfortable with the way I currently look. I'm afraid of the slippery slope. So, the change starts now. If i did it once, I can do it again, right? I'd love to hear feedback from anyone and everyone and any tips on maitenance since that seems to be the most difficult part. Thanks!
