My first thought would be to discuss with your doc and physical therapist what they would recommend since they have the details of your condition, and no one on an internet forum is going to know exactly what your injuries, limitations, etc are.

Aside from that, I would start with much less than 4-5 times a week. I know you're anxious to get going again, I don't blame you. However, if you start out too hard and jack something up, you may be permanently unable to do a lot and that would suck way worse than starting out slow.

Also, don't get discouraged if your strength is less than you anticipated. Six months is a while to be out of it, add to that an injury, and you are probably looking at a significant loss of strength. Don't sweat it though, it'll all come back if you start slow and watch yourself.

Assuming your doc gives you a complete clearance to do whatever you want, you could go from there. Start with looking at your old programs, and how often you trained, at what intensity, etc. I would take into account your previous levels, and lower them quite a bit to get you started.
