Training after an extensive layoff.

Here is my dilemma. I was once a pretty good bodybuilder. then the surgeries started. hernia surgery (which they said is possible to come back if not careful), foot surgery (bunion and fracture), shoulder surgery (shoulder impingement), and the more my body stayed in layoff mode, the more I didn't want to exercise. Please I walk every where and burn off lots and lots and lots of calories. I once had around 30 inch thighs, 17 biceps, to name a few measurements and weighed as much as 188 lbs of muscle. Now, when ever I try to exercise it literally hurts and feels like I have no energy. But I want to get back to my former self. I feel comfortable at that level I was at and my strength was pretty good. Now, I find it hard to lift 110 lbs. My legs are my biggest problem. From those before mentioned 30 inches down to around 21 or 22 inches. What workout would you recommend to me that I can do? Also, I have to say as well, is that I don't eat like I should. I know it. sometimes I just barely get in 2 meals a day. I am currently going to school to be a Physical Therapist assistant which takes up quite abit of time. Can anyone help me get back on track slowly? Thanks for listening.
