New guys needs help!!

Hey guys, so heres the deal i am at university and ive just got back from my gap year where i traveled all over but during this year i have done nothing fitness wise and so becos of this i have lost all my muscle that i had and my tone! (pecks, abs, biceps/triceps)

i have 3 weeks til uni starts so i need help getting back on track before uni starts as i play football and am so outa shape!

so bascially thats what i am at atm! i need to lose the belly and get the abs back, also i need my pecks more defined than they are atm! my arms are least important as im pretty sure i can do a regime for them!

so i know this may seem like im asking alot and i guess people pay for what im about to ask for but if anyone could help that wud be awesome!

i am looking for someone to tell me a regime/schedule for the week to do to get my body back, like how many reps and sets to do at what time of the day etc

also i work mon-fri til 4.30pm so i wud need to fit it in after that, i have alot of gym equipment at my house like weights, fitness bikes, running machine, i do walk to work which is 2 miles to get there and then another 2 miles to get back every day!

LAST THING!!!!
probably the most important thing for me is the nutrition guide, i have no idea what to eat as ive been eating so many diff foods i needs another weekly plan for my food, again what times to eat what and how much etc!

i know ive asked so much and rambled on way to much but if anyone could help me i wud be so grateful!
