Need a routine

I just bought a home gym and next week I plan to start a routine but I have no idea what to do.

I am 120 - 125 pounds right now, 5"11, 18 years old, and I am starting a clean bulk diet next week as well.

My goal is to gain about 25 pounds in the next 4 months, my goal is something I have to meet because I am playing hockey next year and right now I am too skinny (not a lot of muscle or fat).
