Intense Fitness Training - Help me make a plan! Please :)

I'm a 24 year old male.

I used to be very active back in highschool and played on several sports teams. Since drinking in college, growing up, and several years working fulltime I've lost a large portion of my old physique.
Nowadays I weight approx. 195 lbs and my cardio isn't NEARLY as good(although I've been working on it a bit lately and it's getting better). When on the treadmil I typically do maybe 3-3.5 miles in 30 or so minutes.

I've been having a hard time, largely due to work and lack of energy, getting back into the routine of regularly working out. So I figured I'll take awhile off work(so far 1 week but can extend it to 3) to focus intensely on getting back into shape and developing healthy habbits and routines.

I'm looking for help in planning this time off. I want to spend almost every day doing something several times throughout(stength training, cardio, weightloss, etc.), change my diet, or whatever else I can think of.

I've never done anything like this before so I'm looking for a lot of help here, it'd be GREATLY appreciated and I'm willing to put in 100%, report in, keep in touch, before and after photos, etc.

My hope is that after this time off, these routines will become habbit and second nature to me and thus make it easier for me to maintain them.

I'll post again soon and after I've done some research. Any initial thoughts and help making a preliminary plan would be greatly appreciated.
