Help Please

Helo im new to the site and have a question not sure whether this is the right place to post the thread but here goes...

I used to play alot of football sunday league and 5 a side couple times a week and then i moved in with my girlfriend and stayed in watching dvd and getting take aways alot as you do. But i just started playing 11 a side had to pack it in beacause i was so unfit and have put on a load of weight. Started 5 A Side and i h8 playing because i feel so unfit. I Realy want to get back into my football and want to loose some weight or at least tone up and get fitter as i feel realy down about it. Does anyone know a good diet and program for the gym including cardio and weights this would be mutch appreciated.
